Athletic Packet ---
Students must complete a new athletic packet each year.
Before students are able to participate in an extracurricular sport or activity, they must have the following forms completed. They need to be completed every year but only once a year. If you have any questions, contact the coach or our athletic director, Email Coach Butler.
